Activity Request Form Instructions

Linked is an Activity Request Form. Please complete the form for any school-sponsored activity on campus.

The Activity Request Form must be submitted a minimum of two weeks prior to the proposed activity in order to avoid potential disappointments and cancellation of arrangements if your activity is not approved by the administrative team. 

Please submit all Activity Request Forms no later than the Friday two weeks prior to the requested activity.

If you do not provide complete information when you submit the form, you may be asked to resubmit the form and include additional information. Until you receive a confirmation with approval noted, please do not advertise your activity or assume your activity has been approved.

Please note that athletics spaces, the performing arts center, and the library resource center require additional approvals, so your timely submission is critical.

An administrator must pre-approve events where parents or students might have reasonable expectations for a school administrator to be present. It is sometimes impossible to rearrange administrators' schedules to accommodate all requested activities. Advanced planning will help keep conflicts to a minimum (e.g., concerts, performances, etc.)
